    Multiple talents?


    BMI is a widely used method to measure obesity.

    That is, the weight (kg) divided by the square of height (m). The result is that if 18.5 to 24.9 (kg / sq m) is normal weight, 25 to 29.9 (kg / m2) is overweight, while 30 (kg / m2) is obesity.


    This standard was originally designed for statistical tools in public health research.

    Previous studies on a large number of cases did show that when the BMI value exceeds 25, the overall mortality rate of the study population will increase by 29%, with the increase of 5 units per unit. The death rate caused by cardiovascular disease will increase by 41%, while the death rate caused by diabetes and related causes will increase by 210%.


    But with the further research, the limitations of BMI are also obvious.

    For example, although some people have a high BMI value, they are more fit and healthy than many BMI people.

    Another limitation of BMI value is that it can not directly reflect the complications caused by obesity, or the degree of decline in quality of life. These information is very important for the clinical diagnosis of obese patients.


    Besides the BMI value, another common index waist circumference has similar limitations.

    How fat is it to get sick?


    Canadian scientists are trying to establish a more scientific system, the Edmonton obesity classification system (EOSS), for the more accurate diagnosis and treatment of obese people.


    According to the health condition of obese people (BMI > 30), the system was divided into 5 grades (0-4).:0 represented no obvious health problems. 1, there were minor problems, such as blood pressure, blood fat and other criteria, which reached the upper limit of health standard. The body occasionally had pain symptoms, but did not achieve the degree of need for medical treatment. 2 to 4 indicated that there were chronic diseases such as hypertension and type II diabetes.

    The higher the grade, the more serious the symptoms.


    After grading a large number of cases, the researchers found that patients with grade 2 and 3 (as a result of a very severe grade 4 and rarely seen in the cases, which were not included in the statistical results), had a much higher overall mortality rate and cardiovascular disease mortality than those with normal weight. There was no significant difference in mortality between patients with grade 0 and 1 than those with normal weight.


    It turns out that subjects with a grade of 0 and 1 have more than 30 of their BMI, but they are in good health and are not significantly different from those with normal weight.


    Scientists have carefully studied the difference between obese people of different grades. The main concern is their differences in the history of weight loss and lifestyle.

    It was found that obese people with grades 2 and 3 had less exercise and less intake of vegetables and fruits compared with those with grades 0 and 1. The daily recipes of obese people with grades 0 and 1 included more fruits and vegetables and some exercise.


    Interestingly, the study also found that healthy obese people tended to maintain a more stable weight over the years, while obese people with grades 2 and 3 had a much richer history of weight loss.

    Repeated weight loss may cause a substance called high density lipoprotein cholesterol to decline in the human body.

    High density lipoprotein (HDL) is responsible for pporting cholesterol in the body. It can pport cholesterol from arterial lumps to the liver and make it reused, thereby inhibiting the occurrence of cardiovascular diseases.

    If long term weight loss results in a reduction in high density lipoprotein cholesterol, it is not surprising that the incidence of cardiovascular diseases in people with this weight loss experience increases.


    Dr Cook, who participated in the research, said that through the EOSS system, doctors could easily identify who needed to lose weight and who would be good at it now. After all, it is unscientific for all obese people to lose weight.

    Researchers believe that about 20% of obese people have no obvious health problems.


    Although the research itself has some limitations, the BMI value, as a measure of obesity, does not really meet the accuracy of the study.

    Before a more comprehensive study comes out, if you are overweight, but not suffering from illness, instead of losing weight, you will eat more fruits and vegetables and exercise better.
