Notice Of The State Administration Of Taxation On Issues Concerning The Use Of Uniform Invoices By The Manulife-Sinochem Life Insurance Co

Local taxation bureaus of all provinces, autonomous regions, municipalities directly under the central government and cities under separate state planning:

We hereby notify the Manulife-Sinochem Life Insurance Co's insurance business of using the unified invoice of the Manulife-Sinochem Life Insurance Co insurance premium (hereinafter referred to as the "Zhong Hong Life Insurance invoice") as follows:

I. the opening time of the "Zhong Hong Life Insurance invoice".

Since November 1, 2008, the Manulife-Sinochem Life Insurance Co has launched a unified invoice for Zhong Hong Life Insurance nationwide, and the old ticket has been suspended at the same time.

Two. Invoice pattern, specifications and contents

(1) the premium invoice is made of white 80 gram A4 Xerox paper with a specification of 297MM * 210MM (see Annex).

(two) the premium invoice is divided into two parts: the upper part is the insurance premium invoice; the second part is the insurance contract receipt.

(three) the contents of the premium invoice include: "Manulife-Sinochem Life Insurance Co insurance invoice", "invoice number", "invoice number", "invoice date", "insurance company signature", "operator", "review", "business license number", "taxpayer identification number", "national customer service telephone", "company website", "hand opening invalid" and "bar code". In the "insurance company signature" position, the special seal for Manulife-Sinochem Life Insurance Co's invoice is printed, the invoice stamp is left to the left line 15MM of the premium printing area, and the top of the stamp seal font is aligned with the bottom line of the premium printing area.

The printed contents of the insurance invoice are marked in English.

The insurance premium is printed in the form of premium.

(four) the contents of the insurance contract receipt are blank.

(five) the insurance invoice is a single form (invoice couplet). The electronic stub is stored in the computer system of Manulife-Sinochem Life Insurance Co, and the data should be kept for 5 years.

Three. Printing and purchasing insurance premium invoices

(1) Manulife-Sinochem Life Insurance Co branches shall apply to the competent tax authorities according to regulations to print and purchase the invoice of Zhong Hong life insurance premium.

(two) the invoices issued by Manulife-Sinochem Life Insurance Co for other business operations are still in accordance with the original pattern and method.

(three) the local tax authorities should print the insurance invoice in strict accordance with uniform ticket samples and requirements.
